Checking eligibility
- Log in with your mobile number at beneficiary.nha.gov.in
- Search by state, district and ration card/Aadhaar

In Chhattisgarh, state health schemes are integrated with PM-JAY, so most ration-card families qualify.
Making the card
- Take Aadhaar and ration card to a Choice Centre/CSC or the Ayushman Mitra counter at an empanelled government hospital
- After e-KYC (biometric/OTP), the card can be downloaded once approved
- Make a separate card for every family member
At the hospital
Show the card at the Ayushman counter before admission — eligible treatment should be fully cashless. Complain on 14555 if it is not.
